Participation Waiver Form

For many of our AHA, Inc. & school-sponsored and hosted events, submitting a signed and dated Waiver Form is a requirement prior to taking part in these events. This waiver is good for the entire scholastic year.

L.I.N.C.S. - Council Rock Service Learning Program

The new L.I.N.C.S. form has been updated for the 2008-09 school year. It is a now a two-sided form, with the pre-approval on one side and the verification on the other. They are available in the Career Center, Counseling Office, and front reception desk. These forms may also be downloaded from our web site - please be sure to print both pages when doing so. We will continue to accept the old forms throughout this school year.
